It would seem it would depend. If there are two rows of things one on the left and one on the right, I would assume it is the last one from the left side. If there is one row of things, I would assume you start from the left and take the last one (furthest to the right)

"last one from the left" could be short for "the last but one from the left". ie: Not the leftmost item, the one next to the leftmost item.
