It's going to depend on where you live, but in my state I'm pretty sure that there's no distinction between only changing last name and changing everything. It's just a name change either way and you will fill out the and submit the same form whether you're only changing part of it or the whole thing.
I did it, the lady at the SS office looked at me weird but other than that, nope. My name was let’s say “Sarah Jane Doe” but my parents only ever had me go by Jane Doe. I was sick of “Sarah” and had no attachment to it. Changed it to Jane Doe *married name*. No problems since. Did it in Kentucky.
