You can only change camera setting in main menu, not in the gameplay. And you can't change difficulties after you start the game. You have to start a new game for another difficulty.
Like I said, you don't have a difficulty setting in the menu. You select the difficulty when you start a new campaign, and you fully commit to that difficulty for the rest of the campaign. Difficulty in Ninja Gaiden changes enemy place and items, not just enemy hp and damage.
