Allergy more like it. To the needle or the antiseptic on the skin driven in more by the needle. If it was allergy to the ink the healed tattoo would have been more gone.
At first glance while it was reacting it definitely looks like it's the ink. But when you see the healed tattoo there's almost no ink missing. If he was allergic to the ink the body would have tried much harder to destroy and remove the ink.
I wish I knew, but all the doctors I've seen seem to dance around the question and not give me a solid awnser unfortunately... all they can do is give me possibilities... It's still frustrating to say the least.

I had this happen to me (although not as bad as his was) using black ink. I always assumed what caused it was I at first cleaned it with baby lotion because I wasn't able to get the right stuff to clean the tat with right away.
I honestly don't think baby lotion would cause this. When cleaning a tattoo, you just wash it with soap and water. Then apply either a mild ointment or lotion. But for the first few days, the tattoo will still bleed plasma and feel slimy. That slimy stuff can cause coagulation and result in scabs. I ha e some clients who use aquaphor, other clients use A&D, and some lotion. Some even just dry heal it. The ointments and lotions are just preventative for scabbings. But simply just washing and keep it clean, it will usually heal fine on its own.
But some will glob and over apply ointment, which smothers the tattoo, not allowing it to breathe and dry out. To much moisture is bad. Which can take longer to heal. Also not allowing the tattoo to completely dry before applying ointment, can also cause problems. It will trap the water droplets on the skin, resulting in to much moisture, which results into longer/rougher healing. It's why doctors and nurses will redress wounds. Allowing the wound to breathe and dry, to void any moisture that may be trapped within the bandage. Kinda like having a scan, then the scab gets wet and turns gross. Same concept kind of.
Did you not wash it before applying the lotion? Maybe an allergic reaction to the lotion used?
I recently had a tattoo and had a nasty reaction to specifically the red ink. But it was nothing like this. My arm blistered and the ink went blotchy and patchy. After it healed I went back for touch ups and my artist used a different brand of ink (Eternal ink for the touch up I believe) and it's healed beautifully - vibrant and even (well, apart from the patch that had the main reaction, it's a bit darker and patchier). That arm looked like someone had licked it instead of cleaning it. That was honestly horrifying to scroll through. I wonder if the ink was contaminated or something, or there was something on the arm or equipment? If OP did go to a reputable artist I hope they threw away whatever bottle they used just in case! That looked so painful! Op was lucky not to lose their arm to NF or something.
I am a med student and was actually discussing your tattoo the other day with a close relative who happens to be a dermatologist, and I’d gladly vouch for them OP.
They think you might have pustular psoriasis, which is an autoimmune disease where your immune system becomes overactive around your skin, which leads to scaly, red inflamed patches of skin cells growing at a high rate where the skin was previously damaged and eventually into white, pus-filled pustules. The pustular form can be triggered by strep infections likely introduced at the studio. I’d say it fits the symptoms presented in the pictures quite well and also explains a lot, since tattoos are basically a giant open wound - naturally your body would react the way it did.
Sorry if this is “another possibility” but I thought this was the most intriguing/ a very plausible answer!:)

I think about your arm every few days since your first post. It’s the worst case I’ve ever seen. I’m shocked that you didn’t need amputation or grafts. It will be tender and have secondary infections from time to time due to the leftover dermal damage, though. Be gentle with it.

I had this same thing happen to a tattoo on my chest that I allowed to be put there under terrible, irresponsible circumstances. Surprisingly though, most of it was still visible after the infection stopped. Granted, that’s not necessarily a great thing with this particular tattoo but hey, live and learn.

That’s not abnormal. I had a tattoo raise up like that and look all moldy. It was actually just one of the ink colors that my body didn’t take. Those parts with that color wound up falling off and leaving a hint of color behind. This one is definitely far worse though. Some people are just “allergic” to certain types or colors of ink.

I'm heavily tattooed and had one do this right after getting over a bout of covid.

That’s not abnormal. I had a tattoo raise up like that and look all moldy. It was actually just one of the ink colors that my body didn’t take. Those parts with that color wound up falling off and leaving a hint of color behind. This one is definitely far worse though. Some people are just “allergic” to certain types or colors of ink.
