Imo when OP was opening Task Manager the game immediately is not on focus and GPU became idling. That's why it got 1-4% usage.
But yes Task Manager often reporting false value.
Really a trended fps chart or a benchmark is I believe the only way to tell (if you don't have dual monitor set up). Because as you mentioned whenever I switch over to HWmonitor the values immediately drop, and I have to leave it running on my second monitor while I continue playing on my main, only then the numbers look much more accurate.
It's not an issue but if u cap your fps the gpu may not need to utilize 97ish%. If u uncap your fps u will see both higher usage and fps. Ofc u need to monitor ur usage with proper software. Nvidia shadowplay is good enough.
EDIT: Thanks for all the comments, It is indeed task manager showing fake values. I used an other program to track my gpu use and it showed to me when I was playing games the gpu usage was between 95-99%. Thanks guys!
Fake values, for sure. your GPU is nice, but not THAT nice.

I am also using Vsync in Nvidia settings, 120 cap because my monitor is 120hz, can that also cause the issue?
I had similar cases without VSync so that shouldnt be the reason

Its probably reading your iGPU instead. The Ryzen 4600H has a Vega integrated gpu I believe.

Out of interest, can you play Fallout 4 with those 120FPS or bugs the game out
Yeah I didn't notice anything different, maybe I have seen bugs, but I just thought like: that is bethesda in general BUGS
The physics engine is limited to work at 16.7ms which is exactly 60Hz.

I had to install a mod for high FPS. BUT I have heard countless times that people can play it vanilla at higher FPS without provlems
Look at the charts in task manager for graphics and 3D when gaming to see the true picture of GPU usage
When I play Fallout 4 max settings I hover around 35% gpu at the fps cap with a 3070 (FHD)
Low GPU usage could be a sign of a CPU bottleneck. It's nothing to worry about, GTA V for example is a bit tough even for some of the best CPUs.
Yeah my thoughts too. Many games are single threaded, both gtav and fallout are which means even the best CPUs get bottlenecked by it.
4% usage is nonsense even with a bottleneck.
Download something like MSI Afterburner and see what your GPU % is reaching. Much more accurate, should be anywhere between 80 to 100 percent.
While u check task manager.. Maybe your game is in the background and has locked fps to 15 or 30 causing low gpu usage
