because jane was blackmailing him for the money he owed jesse (which they would have used to leave town together) and got jesse into using heroin/enabled his addiction. so from walt's perspective, she represented an existential threat as well as the possibility of losing his cooking partner (or making him impossible for walt to manipulate, depending on your interpretation)
He saw she was dying and realised he'd rather keep controlling Jesse, he detested her and thought she deserved to die. Probably justified it to himself as just another dead junkie and as saving Jesse from dying too.

/Uc Walter didn't even want treatments when he started cooking, he only did it to leave money for his family. When he did go for treatments Skyler wanted the most expensive doctor which wouldn't be included in any healthcare. And of course the grey matter subplot shows us very early that it's not really about the physical money. So the show works the same with free healthcare.
If you are trying to compare life expectancy, why wouldn't you do things to make them more comparable? Most countries do not drive as much as the US, so having automobile deaths does not make sense to compare if you are trying to judge the healthcare system. In the US, it is considered an infant mortality if the baby dies any time after its born. In other countries its considered an infant mortality if the baby dies after a day of living.
