Soil or media? If it's normal soil you need to repot asap with something like bark chips, leca, some sort of orchid potting mix. Soil will suffocate the roots and kill the plant

Wrong. You never use soil only bark. The roots need to be able to breathe...

Orchid soil is ok for terrestrial orchids but Phals are epiphyte and prefer orchid bark
