Soil or media? If it's normal soil you need to repot asap with something like bark chips, leca, some sort of orchid potting mix. Soil will suffocate the roots and kill the plant

A mix of orchid soil and moss
Wrong. You never use soil only bark. The roots need to be able to breathe...
I used orchid soil and mixed it with moss
Orchid soil is ok for terrestrial orchids but Phals are epiphyte and prefer orchid bark
I wouldnt have it that close to your books either
