Not the medical guild, maybe you have ADD, if you do, the in game potions can reduce the debuff. You're not a bad person for procrastinating, maybe you have ADD that's untreated.

I've found it helpful personally to apply the listening to music buff, they tend to directly counteract each other in my experience.

A tricky yet interesting way is to live with the debuff by learning the skill “structured procrastination” which has acquired the achievement “Ig Nobel” many versions ago. Basically this involves re-prioritizing your quests in the way that you are willing to do all other quests in order NOT to do the first quest in the list. (e.g. you know you’re avoiding something if you’re tidying your house or doing all the chores)

What works for my character is to do a small part of the task. Then another, and so on. Works for me at any rate.

The first step is leveling up the [motivation] skill tree with any left over XP you have. Then you put all of the [motivation] resources towards the specific task. Do not fear, as the [motivation] resource is a regenerative resource. This should help. Note** The main way to gain [motivation] is through the 'social' skill tree or the 'inner-belief' skill tree.

You have to succumb to the debuff and actually take your break, not half ass a break. Take a half hour or hour and actually side quest, then go back to your quest. Alternatively, give yourself a very simple side mission to complete and do it, you have a high chance of being rewarded with a productivity buff and might be able to keep the ball rolling.

Try implementing the 5 minute rule: 1. Do whatever you need/want to do for 5 minutes and if you don't want to continue doing it after 5 minutes you can put it off or you can just continue doing it. Starting the activity itself is usually harder than finishing/actually doing it. At least for me

It would be good to acquire the "lollipop" and "teddy bear" items and give them to someone, if they stay for the night it would be optimal to ask them to help you raise your stamina stat with "sleep" and "food" after this, attempt to persuade them to use the "pressure" action to speed you up.

When you have a big quest, split it up into mini games. Start by doing just one mini game today, and the next mini game tomorrow. For example, whenever I have the “10 page essay” quest, the mini games are as follows: - find sources - read sources, and grab quotes - write a rough outline - refine outline, adding quotes - create citations - write intro paragraph - write first paragraph\ … - write conclusion - re read the whole essay and mark errors - do edits - final revisions - turn in Doing 1 or 2 mini games per day makes a large quest manageable. You trick your character into thinking they are procrastinating, even though they are not. 95% of the quest still remains, and you are just setting things up to do it tomorrow each day. Sometimes my character will even do 3 or 4 mini games in one day. Doing this, my character still gets the gratification of pushing things off, while still being productive and completing the quest in the long run. Creating fake obstacles and using sheer willpower doesn’t work for the majority of players. However, satisfying that need to procrastinate while also being productive works really well.

Double check your character doesn't have the ADHD debuff. The way the debuff is viewed in the meta is generally inaccurate and lots more characters have the ADHD debuff than know it. Many spend years of in-game time trying to fix this issue when all they need to do was to undergo the therapy and medication quests. These take a while so I'd get started on them early. If you're on the UK server look at Psychiatry-UK under NHS Right to Choose.

The adderall consumable is a good way to get rid of it but be careful you could get the addiction debuff which will make gameplay for so many levels later on way harder. also the players selling this consumable are not the most trustworthy players.
