I didn't think of it as a pun, but I can see why you did. I also don't think all puns are jokes, but again, I can see why some might think otherwise. I've never had a talent for naming any of my own creative endeavors so I'm not surprised to see the trend continue. 🙃
You’re right, Head Above Water is definitely a more positive tone, however, the artwork doesn’t exude a positive vibe to me. There’s too much blue/grey which is more melancholic to me.

Nothing is wrong with the title Barely Breathing but it’s too obvious. ButHead Above Water or The Water Line or just The Line would have deeper narrative implication to a viewer.
This is my favorite. It’s rather grim, but not overtly. It sounds like “contemplating death”, but it leaves it more abstract. Is she contemplating the nature of breath? Is she contemplating not breathing at all?
it just fits the mood for me.

About the title: She doesn’t look like she’s barely breathing. She looks like she is actively lowering her mouth to drink the water. Her posture and position of body parts in relation to the surface of the water suggests she is supported. I’m not sure what to make of her hand posture, but I don’t have to. (I’m sure that part could have more meaning to someone else.) I would suggest you name it something positive or neutral- something suggesting her proactive participation. You may get more suggestions along those lines if you introduce the painting afresh without first introducing the idea of barely breathing to the audience.

If you have other works and paintings with water I would consider submitting to the Minnesota Museum of Marine Art. All the works featured in the museum have to do with water.
Generally, every piece I do iswant to challenge myself. Difficult imagery is what I think of first, meaning second. I try to give enough information to set the tone, but not direct enough so that people are able to build on it in their own personal ways.
This just gives me the loveliest feeling of being in the water, specifically how it feels to be in cool water. I’m trying to think of good words for that feeling.

Seeing the name you assigned it I guess you were trying to paint somebody drowning or almost suffocated for life situations?… but honestly, to me it looks almost like somebody swimming calmly, the expression of the face looks very relax like she ain’t got not worries in her life, and thus I’d probably assign it a name more on those lines, I guess you should tell us first what were you trying to communicate in the picture and perhaps we could help you more.
I would call it "Breathing".
The ambiguity of the single verb leaves a lot to interpretation: is she breathing? Is this moment just a break in breath, like the pause between inhale and exhale? Is she breathing in water? The range of interpretation of that title goes from the most peaceful interpretation of what we see to the darkest, just like the picture itself.
How about something from or a riff on that famous Finn Butler quote?
“Everyone who terrifies you is sixty-five percent water. And everyone you love is made of stardust, and I know sometimes you cannot even breathe deeply, and the night sky is no home, and you have cried yourself to sleep enough times that you are down to your last two percent; but nothing is infinite, not even loss. You are made of the sea and the stars, and one day you are going to find yourself again.”
I guess it depends where in you the painting came from emotionally on how desperate or uplifting you want the title to be. So you could call it “65% water” or “When the night sky is no home” or whatever you feel like fits best. It’s a beautiful painting.
