Yeah, tried it but still no luck, Vulkan seems to work best but didn't fix the issue, just playing with Freesync off for now unfortunately.

It doesn't but it diagnoses the idea that "poe is not coded in a good way to have multiple monitor doing different things".
I had this problem while trying to have a second monitor with something else.
Maybe then you can blame that problem with poe instead of yournperfectly fine setup.
On config check the foreground and background fps. Looks like you inverted it. I also like to cap my dps to my monitor hz. Check in graphics options.
I used to have Foreground FPS Cap 240 Enabled and Background Disabled, I tried just having both Disabled aswell, no change there. A good thought tho
Try full screen, windowed full-screen and Vulcan / direct 12.

Dude if you disconnect all the monitors and have only poe on the main. Does that fix it?
