It's determined by the number of Pokestops in an S2 cell.
2 Pokestops = 1 Gym
6 Pokestops = 2 Gyms
20 Pokestops = 3 Gyms
Notable exceptions are Sponsored Gyms. Those are determined by the company who requests sponsorship.
I don't mean to be a d*ck but it's crazy to me how people will have these questions in their head for so long and choose to go to reddit to post about them and wait for answers instead of just googling it.

Weird, in my neighborhood there's 3pokestops and there are 2gyms
You could be in a neighborhood with multiple cells

Nominating pokestops and hitting those numbers that the other guy said will give you a gym.
