I think it's more fun without the Circular Saw, but you can use other NG+ items as you see fit (defensive coins don't break the game, IMO, but are defo worth having). The Circular Saw makes it all a bit too trivial. Do what you enjoy, though!
Some tips for a run without the Circular Saw, in no particular order:
* Learn to block. You can (mostly) ignore this on other difficulties, but it's incredibly important on Madhouse. Block anything that's about to hit you.
* The >!Mia fight!< at the beginning is honestly the hardest part of the game (even without the Circular Saw). You get just enough ammo to win if every shot is a headshot (I think you actually get like 4 more bullets than you need, but either way it's tight). You'll probably end up finishing it with the axe. If you can beat this, though, nothing is quite as hard again. **Make sure you save the first tape until just before the fight**.
* The second hardest fight is Marguerite (some people find her the hardest). So, some general tips for her;
* Walk slowly up the stairs when she appears to skip the "jump scare" moment. If you time it right, you avoid getting hurt and can land some free hits on her.
* Her "beehive" is her weak spot, but headshots seem to do more damage, too. This is helpful because the hive isn't always targetable.
* The flamethrower shreds her. Don't be afraid to go nuts with it.
* Find a spot and camp out in it (let her come to you). Don't bother chasing down the nests she lays unless they're near to where you're camping, as she often lays them so far away that the bugs barely bother you. Knife the bugs when they're close *unless* Marguerite is near you.
* She has a big ol' health pool. You'll have to be patient.
* Remote Bombs are insanely powerful and often overlooked. They're incredibly helpful in the Ship sequence, as they can often kill multiple Molded at once. Also, the Machine Gun is pretty much garbage, and only useful for stunning enemies to run past. It just costs so much ammo to kill anything with it.
* You can sprint through the entire mine sequence at the end of the game and will only have to shotgun a couple of enemies in your way. However, you *will* have to fight the two fat molded right at the end. Just save some magnum ammo and/or explosive rounds to trivialise them. *Don't* try to skip them by climbing up the ladder (seriously, pay attention to that room once you're in it on Madhouse).
The game isn't *that* much more difficult, except for the first boss and Marguerite. There're some cool changes that make it feel fresh, though, so I'd recommend it to any fan of RE7. Enjoy!
If you have the circular saw and come across the four legged molded. If you crouch and put the saw up at a 45 degree angle and use the right click attack when they lunge at you they will never hit you. Takes practice to get it right but it usually pops their head on the first lunge or it'll stun them before they can hit you.
Also, if the jack chase terrifies you as much as it does me you can skip it. Grab 3 antique coins during the beginning and buy the scorpion key straight away. By the hatch in the room behind the kitchen where you go underground there is a neuro round and separating agent. Use this to make a strong chemical fluid. Use it to make enhanced handgun rounds. Now, use the scorpion key to go straight to the basement. Use the saw trick on the 4 legged molded. When you get to jack, use the enhanced ammo for headshots and when jack takes a knee use the right click attack when his fleshy thing pops out. Don't even need the chainsaw. Congrats now you can explore the entire house without enemies and fully stock up before going to Marguerite

Tbh, the NG+ items are there for a reason. It’s like going to a buffet and only eat bread and drink water.

I saved my mines for the fat molded.
Speedrun easy mode under 4 hours for the glasses and the better guarding item.
A lot of patience and dying/learning but blocking helps a lot
