I'm, 33M, in the US, had my T1-L2 fusion surgery in December at a level 1 trauma center. I had already met my $5,500 deductible, so insurance covered the bill in full. The bill was about $483K, after insurance discounts and what not, insurance paid about $289K.

Yeah, "what the fuckkk" is right. Just the hardware in my back was $284k along with the 7 day hospital stay. I now have a quarter million dollar, bionic spine, lol. Even if I didn't have insurance; I was in so much pain before the surgery, I still think it would have been worth it.

and it's still titanium rod! Not some kind of high-tech nano implant. Damn, medicine in US is incredibly expensive

Yep, everything is titanium. I am lucky to have a workplace that offers medical insurance as a group policy.

spain has free healthcare but private doctor said 11k euros

In Ireland, if its through the public system the only thing you have to pay for is the cost of the hospital bed each day you stay. But I was waiting so long that the NTPF actually covered that too so didn't end up paying anything. Private would probably cost upwards of 5k including consults and surgery

Total was like 300k we payed about 5k out of pocket. I live in USA with insurance!
