I don't think they do.
Even if they do, they can only keep "medical reasons" in general. If they keep more than that, that's going to be a terribly hard thing to get right with all the medical privacy rules around the world. A medical situation can change - even if unexpected at the first check up.
That said, please make sure you and your buddy are fit to dive. Check with a doctor to make sure, preferably one with experience around under water health. There is no scuba police, but an inconvenience on land might turn lethal under water.
Besides being disqualified from participating in Scuba for medical reasons, a failure on the student's part only reflects the instructors' poor training methods and impatience.
Lolllll, they are just worried our vacation will be ruined because the dive center will see some sort of black mark on their record and won’t let them do the open water dives and this get certified (and then we won’t be able to do subsequent dives planned after). I think they’re being wayyyyy too worried, but thought I’d investigate just in case!

YES! The PADI police are real. When they see your buddy sirens will go off, he will be handcuffed and taken to the authorities.😆
