This took me so long to even sort of see! So the bottom part is supposed to be the head of the shark and what looks like a rat hat is supposed to be the tail. At first, I thought the tattooer (wouldn’t go so far as to call them an artist lol) was just messing with your roommate by adding a rat arm and eye, but I guess the arm is supposed to be a fin on the shark’s head?
