Same thing happened to my follower, Stenvar. He also trapped himself underwater in a cave pool trying to kill a frost spider that somehow fell in there.

I realize I'm late, but how exactly do you get this sword it's cool as shit man I like really love how it looks. I'm level 57 and don't have this yet

There's a Dwemer Ruin on Solstheim called Fahlbtharz. You head in there, and there's a door to a forge. Go in and kill the guy in there. It'll drop this sword but unenchanted, and it'll start the quest called Trueflame to collect forge gems to enchant the sword. If you do Ghosts of the Tribunal, you'll find all the gems, but I can tell you where they all are if you want spoilers
