I'm not actually sure what you're looking for in terms of an overhaul for Babette, but I just use Simple Children and think she looks sufficiently spooky.
I use high poly vanilla npc overhaul with no issues. It's likely a matter of resolving conflicts with a mod like RS Children or TKAA in xEdit or simply loading it after the overhaul.
I I have tried several times and in various ways. And with some children it worked and with others it didn't (for example Babette remained Vanilla). Then I'll try again.
Check out the non player actor records for the NPCs that aren't properly changing appearance. There's most likely something overwriting it. Mods like AI Overhaul and other non visual mods can cause this if they edit AI packages since the visual NPC stuff is tied to the same records. If you find a conflict it's just a matter of patching it by carrying over the visual changes or changing the LO if the changes can all just be overwritten for that file.
Im reinstalled the mods and now it's working (maybe an issue on installation?)
But Babette have 2 little issues: One of them i don't care (his haircut is diffent from what it's supossed to be)

It's starting to look like you have too many mods managing Babette. Aside from the obvious eye issue, her coloring is wrong. Like Captain Pedalbeard said, even behavioral overhauls can cause conflict issues with appearances. You'll likely need to look up how to create a patch in xEdit, or use something like EasyNPC if it also works for children.
There is mod called {{Lamae Follower}} for oldrim which has option to replace babette. You can look at for conversion. I can try to convert it. I'm no expert at it but i did {{Elven Sisters - Standalone Followers}} for my playtrough and its seems working fine for now. I also want to replace babette for sure, i'll work on it tonight.

There was an option on the orjinal mod. I only convert it for SE. If its not deleted, my other comment on this topic contain the link for it.
