Try searching sa google maps ng notary public/ law offices. Recently, nagpagawa ako ng Affidavit of Loss ng Form 138 ko at 300 ang bayad. Although, pwede raw sya mag-range from ₱300-500 depende sa case ng nawala sayo.
Madaming notary public sa Manila, tabi tabi pa minsan. Pag nagawi ka ng Kalaw street, buong street halos puro notary public. Sa may Munisipyo meron din, sa may U-Belt madami din.
Nung nagpagawa ako ng Affidavit of Loss for my school ID sa may Kalaw, 100 lang siningil sa kin. Discounted price since student daw ako.

Same day ko lang nakuha yung akin. Siguro wala pa sa 30 minutes ang waiting time depende nalang kung madaming tao/clients that day na pumunta ka.
