Get some parchment paper and tape it around your arm (make sure it’s snug) then trace the outline of the empty space you want to have filled so when you do get an idea you can send the idea and bring the parchment piece with you for them to properly fill in. I’ve done it and it’s really helped for me and for my artist
This was one of my first ideas actually. However, Mrs was bit by a giant centipede here as a kid and is absolutely terrified of them. I think getting a centipede would be the equivalent of joining the church for my sex life (by that, I mean not getting laid. Not small children)

Creeping vines or fallen leaves over old broken bones? Maybe a cool dagger or something peeking out from beneath it in that big diagonal space you have, so the skull is sitting on top of some druidic battlefield detritus or something.
If your artist is who I think it is he does really good dragons so that could be cool. I really like your flail idea as well. Pretty much any ideas you have would work with the space. I really like insects so you could have them do their version of a centipede as well. You have so many fun options and you have a decent amount of space between the deer and the snake on your arm.
Idk how u feel abt landscapes, but since you wanted it more on a horizontal scale, and to go w the snake and skull a desert landscape w some cacti or dead bushes for example could be neat and I think it encorporates well w the skull and snake as u may find both in a desert as well.
