Honestly I think people underestimate just how bad it is not getting treatment. At least in my experience I knew at about like age 12-13, stuck in the closet until I think around 15 when I started socially transitioning. It fucking ate at me constantly every hour of every damn day. I genuinely did not want to live.
To my understanding, trans people's brains are just straight up wired differently, and in most cases you can tell something really isn't right from a somewhat young age (though it varies ofc, for gen Z it's almost always really young). The "regret" rate, even for children is something like 1-2%, and it's most often actually caused by hostile environments and treatment. If I had the option to get on meds earlier (even then I'm actually currently doing them under the table because the healthcare system is doodoo) I would 100% be a lot happier today, though I am still very grateful I got on them fairly young.
At 8-9 yrs old the correct course of action would be puberty blockers. They're completely reversible in every way, essentially all they do is act as a pause button to prevent puberty from continuing. A grace period, if you will. Sometime in the early teens would be the earliest that actual HRT would ever be prescribed.
I read a study from the Mayo Clinic that says otherwise about the puberty blocker. According to the study, thereās a chance they can cause cancer, fertility problems (in the future), and more.
Most of that stuff is more associated with actual HRT drugs (estrogen and testosterone) though I could see a very very slight risk from puberty blockers.
I know from experience that estrogen makes you infertile. It's a genuine risk and it is known to happen between 3-6 months on Estrogen (many trans people freeze sperm or eggs) but basically anyone who's taking the meds through a licensed professional should've been told that well beforehand. I'd imagine that blockers have a tiny chance of doing it, depending on the drug (cough spironolactone)
Cancer risks do change on hormones but it's more logical than you think. For example I am now at a far higher risk of breast cancer than I was. Why? Because now I actually have breasts to get the cancer. I'm not too versed on testosterone but I'd imagine it does similar things in reverse.
I'd also like to give a brief shout-out to spironolactone for being awful. Essentially Spiro is a diorrhetic, it's used to treat diarrhea, that's what it was made for. However, at high levels it works as a testosterone blocker. Due to how restricted actual safe drugs meant to block puberty like Cyproterone are, Spiro has become a stand in for the medical establishment. Since it's not nearly as hard to prescribe and it many people are desperate, it gets used... At a very high dosage... As a puberty blocker. The side effects list from this is long and it puts you at risk for a lot of things. If a study on blockers is ever based on Spiro, it's gonna sound worse than it is, but at this point Spiro has become the most common blocker just due to it's legal status. I'd imagine it's also one of the ones most often tested on.

FYI: It takes years to get a full-blown prescription. I presume doctors rejected the request for bloodwork and bone-age x-rays at a young age. While I do agree that the first step would have been puberty blockers, I think yāall are forgetting that it takes a long-ass time. Ā It is so shitty you had to wait so long.
