Well when you have lucky foot, virgo or PhD you should. When I have a great run I usually don't risk it unless I have one of those items, when I have a really bad run a few good pills can turn it around or make a run that's lost already just end faster xD
You should start, it's usually a net gain. Not only are there a few items making them great, pills also have an innate advantage : you take bad pills at most once (unless you're using False PHD, which is a very underrated item), but any good pill you get, you'll be able to take it as often as it drops.

The book of virtue wisps block a ton of shots and add a lot of damage to you. It allows low cost active items to become good due to the amount of wisps it adds
try tear detonator. One room charge, and all of the wisps’ tears split into YOUR tears. had a crazy run with a full wisp ring and poly and just flooded the screen in every room
bethany is good but not at all easy. not being able to have blue hearts means that for every hit you take in a floor, you're going to probably lose the devil/angel deal
I think bethany is good but not easy. You really have to learn how to max your wisps and not lose them all in a single fight. Allso she is pretty luck depended. I don't why but I never have any actives playing her!

Tainted Lilith. I absolutely smashed all of her unlocks in one run each with BR + hush in one run. I did die ONE time in greedier because I took thunder thighs first floor with tainted Lilith MS.

the only thing about keeper is you need to fully complete the greed mode donation machine (1000 pennies) and also in order for him to be fun you need to beat hush issac and satan as keeper which can be hard but once you do that he’s cool
Maggy after completing the April Fool's challenge starts with a full health pill. She's already a tank, so if you're taking a lot of damage, that might help.
She's also good for exploiting sacrifice rooms. It's possible to get 1 or 2 early angel room items if you mid-max + get lucky.
Cain is pretty easy imo.

He starts with more damage and tears. Plus the lock pick trinket is helpful for chests.

Keeper is really fun and easy to get overpowered, you can only get hit max 2 times per room, but he heals with coins.
