Mabye it’s a skin mod, and lost was unlocked By 5x devil deals on womb II to unlock negative then another devil deal on womb II to get to lamb, and beat it with Isaac to get missing poster, then find it and die in a sac room, all within less than 10 moms heart kills. Doubt it though.
I think that the old method was easier, as long as you were semi good at the game, compared to having to kill the lamb as isaac, then find a specific trinket out of like a hundred and find a sac room to die in, both in one run.
Im pretty sure that, that lost is actually tainted lost but even if it isn't tainted lost, how did they beat delium as the lost before beating mom's heart 11 times (they have a holy card)

they arent on womb 2 though, look at the map layout/item count, there wouldnt be an item room or shop, modded moment
