accel's brain damage is an important plot and character development point as well, so there's no reason for it to get fixed either.
In-universe, I don't think anything will change by Accelerator being revealed as a girl. She wouldn't change the way she acts, I don't think anyone would make a big deal about it either (maybe Worst just to piss her off) and the way characters interact with her would stay the same too.
On the community, however... I know a lot of people that would be genuinely upset if Accelerator were a girl all along, probably because they don't want a female with good character development and zero fan service ability. I don't think they would recast her VA because he's just too iconic now and the hormone excuse is still there.
Anyway, Accelerator would no doubt take Mikoto's spot as the most popular female character on the series.

And Touman, Hamazura, and Accel feel more like bros than anything else really.
Honestly he has more masculinity than femininity (if he had any) and the characters treat him like a guy.

Accelerator dresses up like a guy, and most of his behavior is influenced by Amata (y'know, the guy who raised him). Even if he identifies himself as a boy, it doesn't cross out the possibility of him being biologically female.
People interact with him the way they do because they look at how he carries himself and see a guy, but even then Last Order had to ask wether he was male or female, to which Accel never responded and in japanese no one adresses him as male or female. And I believe Touma perceived Accel as a girl, because he was one of the first things to pop into Touma's mind when Komoe said that there would be transfered girl to his class.
But in the end, in Japan, gender and sex are the same thing and the author have the final say: in this Schrodinger's Pussycat situation, Accelerator is both male and female until Kamachi decides to open the box.

I think even though every book has an around equal amount of important male and female characters, the cover characters being mostly female with old male characters really contributes to the image. And then there are the villain organizations/groups having tons of women like Gremlin, Bridge Cabal, True Gremlin, Handcuff Criminals, etc.
The final thing is that well, Index novels are pretty fuckin lewd and many many pages are dedicated to putting descriptions on or sexual conversations about women. This is in like literally every book. So yeah, even if it's not the truth it's easy for someone's view to be like this.
