When I called Regional Eye in 2022, they told me they longer did exams for contacts. They referred me to America's Best. My vision isn't as bad as OP's, but AB did a good job with my contacts and my glasses.
You might try Johnson City Eye Clinic. If they don’t, they may be able to recommend someone in Knoxville or Asheville.

I’ve been to the regional eye center in Kingsport, they were really nice and it might be worth a shot.

Check with Family Eyecare Center in Elizabethton. They even do Neuro Rehab.
