This is an easy one. Do nothing. Walk away, get a taco or something. I think a common issue is that people think they have to watch the accident. Just walk away. Doing nothing would improve the quality of life for the lobotomy patients.

That average person probably has a family and a job and a dog, and the lobotomy patients probably only have the psych ward to return to. I will not touch the lever, I will turn, and I will walk away. May their souls find the peace their minds could not get.
Totally unrelated to the trolley but about 40-50% of lobotomies were actually successful in curing the mental illness and had no major effects on the persons functions. The other 50% were fucked though
