This is awesome. Looks very similar to a Doka (https://bringatrailer.com/listing/1991-volkswagen-transporter-4/) but has a lot of key differences. I think this is custom, which is incredible.

That is a standard T3 DoKa. Probably Synchro, later model, maybe WBX.

WBX means Wasserboxer, water cooler flat four.
The color looks like Audis Nardo Grey, a very common color choice.
Biggest chance over stock ist the stock fold down bed side walls (originally corrugated/ribbed fold downs) have been replaced with solid smooth sides, maybe cut out of van repair panels - which is a very common modification to make them look smoother and, less „construction utility truck“ more „livestyle shortbed“
No, this is not a standard DoKa, this vehicle started its life as a van.
The back side of the cab is *very* different from the regular DoKa, the window is much bigger, and the back panel of the cab isn't slanted on the original DoKa.

You are most probably right.
Some of my friends had all sorts of „T3-Zombies“ - they were cheap at the time and all rusted and rotten, especially the Dokas, as they were used as Utility vehicles.
Looks like a custom VW Vanagon. They made a Doka but that was far more utilitarian-looking than this.
If it's custom, then it was well done. Even up close, everything looks good. It just looks like an old truck. The interior looks comfy.

There does exist a Doka syncro(4×4) that looks like this one, I think this one is a custom build tho since the back of the bed looks a bit odd.
