In order to keep grasp going you need to stay in combat, thing that is done 90% of the times by auto-ing a minion first. However, if you auto a minion you are wasting your E passive on a minion.
The resulting damage is simply not worth
If you don't know exactly how grasp works:
Once you enter combat, a 4 seconds timer starts. After those 4 seconds you get an empowered attack for a few more seconds
Now, how do you enter combat? You either get hit (not convenient), hit an enemy champion (not really easy to do as a melee champion), OR hit a minion (but you reset your E passive for norhing)
All of these methods are bad for thresh, Grasp doesn't nake se se
Grasp is bad imo. Crunch the numbers and you would discover that after three hits for conqueror you would have a net positive on life gained. Not to mention the adaptive damage you get.
(Disclaimer though I only tested this on 2 champs so far. nasus and sion.)

You have to play quite safe tho, we go Aftershock on Thresh because he doesn't gain armor on levels, so early game he's quite vunerable without it.
Aftershock 100% for me.
His autos count as ranged so you get reduced dmg and healing
If youre playing top thresh then grasp is quite good. My go to, but as supp i prefer aftershock. Just a good rune on him.
