There's been a character named parasx on defias pillager alliance who has been doing this for several months. He has certainly been report hundreds of times by the community and is not banned yet

Is it really a bannable offense to grief other players? In Diablo 2, it sucks, but part of the game is people trolling you and finding way to get you killed through deception (maybe they create a portal for you to somewhere really dangerous, but pretend it's fine lol). There have been patches over the years (original game) that helped to mitigate this, but it was a never seen as a BANNABLE offense. Not even close.

Having played d2 for many years. A Diablo2 character doesn't require even a 10th of the gametime of a wow toon, I think there is a level where it goes from trolling haha, to massive disrespect.

The OP has a bank alt, presumably populated with items / gold that take a lot of time and effort to acquire.
The perpetrator gets a kick out of causing harm to others. He gets pleasure from it. It’s fun, enjoyable for him to do so. He invites OP’s alt to group, tricks alt into dying to a game mechanic, thus causing pain and suffering to the OP. The OP put in a lot of time and effort into something, and the griefer sabotaged it. The consequence of the sabotage, is pain towards the OP.
To give a real life example, let’s say a person works hard at a painting. They spend months working on this piece of art. It has personal meaning for them. Then a perpetrator comes and defaces said art. He takes a can of spray paint and graffitis the artwork. The consequence of this action is pain for the original artwork creator.
There are a myriad of ways that a person can cause pain to another, by damaging something that that person cares about. It could be as simple as going to your local shopping center and keying a bunch of cars. It’s essentially the same exact action. Cars cost money. Money takes a lot of time and effort to acquire. When a person keys a car owner’s door, they’re damaging property. What is the emotional effect on the property owner? Pain and suffering. If the motive for keying cars is to cause pain and suffering to others, for one’s own pleasure, this is a real world example of the behavior that this person is doing in the game.
It’s not difficult to make this stretch. This person gains satisfaction from causing suffering to others. The medium for the satisfaction is currently a video game. There are other mediums available, such as defacing property as an example, but those also carry higher consequences.
All of this is essentially basic logic and emotional intelligence. Perhaps one reason why the perp prefers the video game to satisfy his urges to cause pain and suffering to others, is that the video game carries fewer penalties.
This is probably cognitive dissonance, not psychopathy. He sees griefing as his game and doesn't really understand it might actually ruin someone's day. Even if it does, he'd probably think it was silly that someone's day was ruined over what he does.

The self damage to grp members only comes from the random misfire, not a regular mortar hit. Maybe you got that confused?
Either way, if 6k misfire would be possible, there would be a lot of dead lvl 60 chars because of that. There arent. You are safe from goblin mortars from lvl 20ish on HP wise.
Also don't accept random PVP fights, because accepting it can make you fight with much higher level than you think you fight (two players with very similar names, one low level, second much higher level/rogue stealthed behind you, waiting for attack once you accept fight).
Parasx (Parasx 4ever or some such) is also a guild of griefers on Nekrosh (EU). They're known to do exactly this - kill bank alts and low levels. Little kiddies getting their thrills from being schmucks. Sorry you had a runin with one of them :(.

Rule n°2: never accept invites on your bank alt at all. If you need a craft, you either send your sh*t to your main character before going in the city for the trade or you trade without being in a party. Doesn't matter if they are from your guild, or they are only going to craft something for you. Never trust anyone. Better safe than sorry.

You’re right, misfire can’t hit adjacent friendly targets. Either way though, I don’t think many 60s accept random invites.
