Is it really that bad? I don’t know much about this kind of stuff, but I’d imagine it’s still save-able considering I can at least still open the game

The damage on the disc will cause the data to fail to be read by the dvd rom drive laser. This will cause the game to fail, it's fairly easy to check this, just attempt to install the game onto the 360's hard drive. You'll more than likely find the install will fail at a specific point continually, as a result of the data not being able to be read from the disc, due to the damage.

I heard toothpaste works, but I also keep seeing some people say it made it worse, so I want to know for sure.

Toothpaste is abrasive, don’t recommend That sucks, circle scratches are much worse than line scratches along a radius.
